Mptp taxi card application form doctor

The Multi Purpose Taxi Program ( MPTP ) online form is for doctors only. Please carefully assess your patient against the medical criteria in the MPTP online form. Please complete the online form and submit electronically.

The consent form needs to be completed by you and retuned to CPVV. Once CPVV receive both forms, please allow working days for a decision to be made. If your application is successful you will receive an invoice for your MPTP Card.
